Controls by design/default: collect less, limit access, explain decisions, allow reversals and fallbacks.
Oversight & decision rights: when a human must review; who can overrule; how to resolve conflicts.
Evidence pointers: links to logs, samples, notes, and user messages.
Transparency note (3 lines): what changed; why it helps; your choices or how to appeal.
Five practical moves for this quarter
Decision log (1 page): the choice, the options, the reason, the risks, the owner, and the review date.
Human checks at key steps: test quietly first (“shadow test”), then ‘spot‑check 5–10%’ of real results where stakes are high.
Overrule with a reason: if a person disagrees with the tool, they can change the outcome—but must add one short sentence explaining why. Record ‘polite disagreement’ even when the result stays the same.
Plain‑language “why we said no”: if an automated decision harms or blocks someone, explain it in simple words and show exactly how to appeal.
Emergency stop & quick undo: have a clearly marked ‘stop button’ and a fast way to roll back if things go wrong.
